Clapp said Bond was “having trouble adjusting her bra holster, couldn’t get it to fit the way she wanted it to. She was looking down at it and accidentally discharged the weapon.”

From the article:Clapp said Bond was “having trouble adjusting her bra holster, couldn’t get it to fit the way she wanted it to. She was looking down at it and accidentally discharged the weapon.”
This is - most likely (we currently don't know, as I have not yet seen any pictures of the holster that this particular person had been using at the time of her demise - not a case of the holster failing.
From the way that statement reads, this is operator error, pure and simple.
