What can you if the sheriffs you went to doesn’t send your CCW license within the 45 days required of them

Not really a whole lot.

First, call them, if you haven't already, and remind them of the requirement under the law, and that it doesn't give them any exceptions - not even if BCI is slow returning the results, or a county in your history is slow, etc. They can always revoke the license if something eventually turns up.

Next, try to talk to the actual sheriff - they may be unaware their clerk isn't following the law.

Finally, you can start the process of filing a writ of mandamus - basically, a court order to do their job under the law. This can either be an actual filing, or a threat - say, when talking to the clerk, when they say they have to wait, ask them to transfer you to the clerk of courts so you can file a writ of mandamus to compel them to issue the license... No public servant wants to have a mark like that on their record.

Having the same problem in Pickaway County sheriff says its BCI fault and the officer in charge of CCW at PCSO says BCI actually hangs up on her when she calls about delay,been since june for me have heard nothing but BCI at fault
