Change of Address

In 2015, while living in Geauga County,I acquired my license in Lake County. Now,I'm living in Portage County...Where/how do I change my address? Geauga Sheriffs Dept. had no answer for me

You notify the Sheriff in the county that issued you license - Lake County. They may take the change of address over the phone, or through other means. Check their website, then call them. Or, just send a certified letter, return receipt requested and be done with it.

Edit: If it has been more than 45 days since the move, I would not mention the date of my move in the letter...

I would keep it simple, like this...
To: [county] County Sheriff' Department
Re: Required notification of change of address

John Doe

888 Broad St
Cincinnati, OH 45678

Nice sample letter, MWSY.

Oh, and yes, I know that a certified letter isn't proof of *what* you sent in the envelope. However, as it will be sent from your new address, I think it could easily serve as sufficient proof of notification of the change of address in the unlikely event that becomes necessary.
