Failure to feed??
Can anyone explain what would cause this???

This is a factory load pmc .223 rd shooting out of rra elite operator 2
Also this is second time this has happened locks the bolt up first time had to pry it out
This time was able to yank on charging handle

IDK. Too soft brass maybe? Looks like its getting slammed at the point of extraction.

Is this happening in the first rounds of the mag? Dust cover closed during first round fired? variables...

this time it was first mag but third round
Last time many months ago after several mags +running 22lr threw it then back to .223 I believe it was first mag third round

How many rounds down range on that extractor spring? Have you torn down the bolt thoroughly to inspect the spring ect?

Poor quality control at PMC. I had a similar case last year from another manufacturer. I saw it before it went in the mag.
